You will automatically be given an open account with your first case. For cases, simply send us your prescription with the patient's models. Mailing supplies are available upon request.
Cases are invoiced when they enter the lab and the amount is automatically billed to your account. When the case is completed, it is returned to you with itemized invoice. On the first day of each month, a statement summary is mailed which totals the previous month's invoices. Statements may be received shortly before your order arrives. All accounts are due on or before the 15th of the month. A 0.5% interest charge will be added to all account balances over 30 days.
We accept Visa, MasterCard, Discover and American Express.
Normal laboratory time on a new appliance case is 5 working days (This does not include the time the case is in transit to or from the lab.) If your patient has been scheduled, please note the day before the appointment date on the RX slip as your due date. If there is a problem delivering the case to your office on that date, you will be notified.
Rush Service is available if needed for an additional 25% charge.
You will not be charged for a repair to an appliance within 60 days of the initial fabrication. All other repairs will be made for a nominal charge on any appliance returned in repairable condition with the appropriate models and wax bite. Repair and replacements for broken or lost appliances are provided for under our insurance plan which is available on most appliances. Repairs turnaround time is 3 working days in the lab.
Orthodontic Technologies supplies postage paid mailing labels for your convenience and provides return shipping at no charge. All packages will be returned UPS Ground, 2nd Day Air or 3 Day Select unless otherwise specified. Overnight delivery is also available upon request.
