Fill out the prescription slip as completely as possible. Have your name, address, phone number and patient's name written clearly in the space provided. Remind your staff that good models are the most important step in creating an appliance that fits well. Pouring models and inspecting them before the patient leaves is always a good practice to follow. Not only will it save you the time and expense of retaking impressions at a later date, you will also avoid treatment delays. Proper packing of models is very important. Inadequate wrapping/packing may result in breakage during transit. Broken models will delay treatment.
Normal laboratory time on a new appliance case is 5 working days (This does not include the time the case is in transit to or from the lab.) If your patient has been scheduled, please note the day before the appointment date on the RX slip as your due date. If there is a problem delivering the case to your office on that date, you will be notified.
Rush Service is available if needed for an additional 25% charge.

Please verify that the impression is free of defects around the teeth. to ensure a perfect fit
Make sure that each alginate impression is packed with moist cotton gauze or rolls, lightly wrapped in a moist paper towel then packed in a plastic bag to retain moisture and preserve the material
Please make sure arch wire is removed when taking the impression if you require the lab to carve the brackets. This will insure a more accurate model. Verify that models are free from distortion and any imperfections that will affect the fit of the appliance. If there is a problem that we notice, the lab will call for a new model. During shipping teeth are often broken. Models should be wrapped individually and be secured tightly in a sturdy shipping box.
